In 2023, Australians lost an estimated 4.4 million years of healthy life due to chronic conditions, equivalent to 85% of the total disease burden in that year*.


Earlier detection, diagnosis and referral by primary health clinicians is critical to delaying development, improve quality of life and reduce potential demand on other parts of the health system.

Yalchin Oytam: Yalchin Oytam is the founder of Data Revolution, a cutting-edge data science consulting firm consisting of data scientists, engineers, visualisation experts and a business consultants.

Previously, Yalchin led South Eastern Sydney Local Health District’s data transition and analytics strategy. Under his direction, the LHD ranked first in the state-wide transition of NSW Health’s primary data repository (EDWARD).

In 2023, Yalchin was recognized among Australia's 'Top 10 Analytics Leaders' for his ground-breaking work in health data science, with invitations to speak at conferences in Australia and the US.


Josie Di Donato: is the Project Manager for the PHASES with Primary Sense project at Country to Coast (CCQ) Primary Health Network. Josie has a background in health information management and has many years experience and held senior roles in Queensland across primary care, hospital care and academia. Josie is dedicated to strengthening our healthcare system through purpose-driven digital health projects that effectively support clinicians and deliver significantly improved patient outcomes.


Tim Garden: With more than 20 years in healthcare, Tim is passionate about the use of innovative digital health capabilities to unlock new models of care and to enable the delivery of high quality, integrated health services.

The CCQ Clinical Societies are a platform for all health professionals from the region to come together to make friends, network, learn new and localised information and have fun in a collaborative, knowledge-sharing space – and earn CPD points!

Health professionals, including GP’s, nurses, psychologists, mental health workers, Aboriginal and Torres Strait Islander health workers, physiotherapists, occupational therapists and other allied health professionals and hospital clinicians.
