🌟 The Care Manager's/ Partner's Guide to Budgeting and Integrated Service Delivery under Support at Home
Due to multiple requests from you all we are excited to announce our in person training workshops specifically designed for care managers.
As the aged care sector evolves with the introduction of the Support at Home Program, care managers and partners face new challenges and opportunities to meet heightened expectations.
Care Managers/Partners will need to know:
· The new scope of practice for their role- what is new and different?
· What are billable and non-billable tasks
· How to manage a quarterly budget- (very different to the way budgeting is done now)
· How to manage current/new and transitioning participants
· How to manage a flexible case load within a care management pool of hours
· How to ensure the risks and vulnerabilities of participants are managed
· How to document efficiently and effectively
LPA was there for you when Consumer Directed Care was introduced in 2015 and provided training to care managers across Australia in how to manage consumers budgets and the requirements of Home Care Packages. Remember the USBs? And the laptops in our training sessions? Fast forward 10 years, and again, we want to ensure that you are supported in a practical, easy to understand manner so that your care managers can confidently embrace the Support at Home changes.
We are looking forward to hosting these in person events, delivered by LPA’s managing director, Lorraine Poulos and supported by the LPA team. LPA is here to ensure that providers are supported now and into the new future of aged care.
🔥 Overview
Care partners will develop a deep understanding of the Support at Home framework through this one-day workshop and acquire the skills to lead meaningful change within their teams and organisation. Designed to bridge the gap between policy and practice, the session offers a mix of expert insights, case studies, and actionable tools to ensure care partners can deliver participant-centred services while maintaining operational efficiency.
The workshop provides attendees with tools to address current challenges, including navigating the complexities of billable hours, ensuring they are well-equipped to deliver future-ready, participant-focused care models that optimise both time and resources.
🚩 Learning Outcomes
By the end of the training workshop, attendees will be able to confidently:
1. Insight into Evolving Care Management Practices: Understand what is new and different about care management in the Support at Home Program and how these changes impact service delivery e.g. the three service streams and the possible impact on service delivery.
2. Proficiency of Caseload Management: Learn strategies for effectively managing caseloads to balance quality care with organisational efficiency.
3. Preparation for Current and Future Needs: Explore what is required to meet the needs of both current and future participants, aligning care with the program’s expectations e.g. contributions, quarterly budgets.
4. Skills in Outcome-Based Care Planning: Develop the ability to write care plans that emphasise wellness and reablement, ensuring consumer goals are at the forefront.
5. Integration of Budgets and Needs: Review the four cohorts of participants in Support at Home and the unique budgeting that will be required for each person. Discover how to connect a participant’s budget with their care needs, fostering financial sustainability and personalised service delivery.
6. Recognition of Clinical Needs: Learn to identify when a participant may require the support of a clinical partner and how to integrate clinical expertise seamlessly into care plans.
